I run into this stuff a lot. But I write up a ton of data for my job.
After 3000 pages of development notes to my boss, you learn to spot weak notes fast.
As a development grunt here in town, you have to create filters against this.

This short list a nifty tool for non-industry folks.
The kinds of notes discussed are more indicative of the reader, than the writer.
And if misinterpreted or sent down as a decree from on high, it's toxic for the writer.
